Yamamoto always jokes about how playing mafia with Tsuna and the others is really fun. The pop guns, the wooden swords, plastic knives and so on. Yamamoto doesn't seem to think that the battles they fight are anything more then a kids game. Personally however I don't think Yamamoto actually believes that. I'm sure in the beginning it seemed as if it was all fun and games however sometimes during the Kokuyo or Varia Arc Yamamoto must have realized the truth. Afterall to go through a life or death experience and brush it off as a game? That's pretty hardcore.

In the Varia Arc he learns how to use a sword for real and goes so far as to say that he wouldn't kill Squalo. If he really thought that it was a game and that it wasn't a life or death situation why would he have tried so hard and fought for so long to win and then refuse to kill Squalo. If he didn't believe that he was in a life or death struggle why would he refuse to let Squalo die? Overall Yamamoto still jokes around about it in the Ten Year Later Arc however it is severly obvious that he knows the truth and had known for a while.

I'd like to see when exactly Yamamoto realized in a flashback or something but so far Akira Amano kind of refuses to say. Hopefully it will be discussed in later chapters. Since afterall, Yamamoto is important and a fairly main character. So he should hopefully get a chapter about it.
